Program Goals: 2012 Wellness Center Goals Goal 1: Continue to focus on Peer Whole Health. 2012 Northern Arizona Consumers Advancing Recovery by Empowerment, Inc. Board of Director Goals NAZCARE Board of Directors will: Goal 1: Establish gardens in as many NAZCARE facilities as possible. Objective 1: Board Members will assist in building, working in and securing materials for the gardens at each site. Gardens in place: New Hope, Next Steps, Discovery. Objective 2: Board Members will help to manage and market gardens and produce from the gardens. Goal 2: To promote Membership from each center on the Board of Directors. Objective 1: Board Members will contact other centers and recruit qualified Members to participate on the Board. Objective 2: Board Members will assist to recruit by contacting other centers and Center Managers regarding participation even through Web-x. 2012 Training Goals Goal 1: Produce Quality Distance Learning – Media Training. Objective 1: Upgrade Mind Flash’s NAZCARE Training Series • Outcome: To have professional quality Mind Flash trainings. Timeline: Beginning May 2012. Outcome: • Upgrade Mind Flash for internal training • Upgrade Mind Flash to market to outside training and as a marketing tool. Goal 2: Provide Peer Employment Training to Tribal Entities Objective 1: To outreach training to Tribal Entities in NAZCARE Training Series. Outcome: To market Tribal Workforce Development Model to tribes in Arizona Timeline: on-going March 2012 and continuous Objective 2: Provide Support Employment to Tribal Entities Timeline: Beginning in March 2012 and on-going and continuous market the workforce development model to Tribal Entities. Outcomes: • 4 presentations were given for tribal entities ( TO presentation March 15) Objective 1: Place recovery first by using evidence based practice of Peer Whole Health. Outcome: Better Wellness among Members and a better awareness and understanding of recovery. This will be measured through goal achievement. Objective 2: Begin to incorporate health measurements in the steps to integrated health. Outcome: Every Member will have the opportunity to check blood pressure, temperature as part of a daily wellness check and record wellness. Goal 2: Help members to provide one to one support to another member in whole health: blood pressure hypertension, cholesterol, diabetes and weigh control and maintenance. Objective: Educate Members about the importance and need for support in recovery and whole health and how these are linked and related. Outcome: Assist consenting Members to have weekly Peer to Member support among members with or without assigned topics in order to support each other. Members may elect to have peer pairs for better one to one support..
